Please note that the survey software may not work with
all Internet browsers. If you receive an unexplained error
please try accessing the survey using a recent version
of Internet Explorer. Also make sure your security settings
allow for "Java-script" execution.
If desired you can temporarily lower your security settings in Internet Explorer
by selecting the TOOLS>Internet Options>Security tab and then lowering
the "slide bar" downward to a "Medium" setting. You can reset this security
option following completion of the survey.
